NAD+ serves as an electron carrier in the electron transport chain, cycling between its oxidised (NAD+) and reduced (NADH) forms โ the fundamental mechanism of cellular energy production. NAD+ declines primarily due to increased consumption by CD38 and PARP enzymes โ not reduced synthesis. CD38 (a NADase) increases with age-related inflammation. PARP-1 is activated by accumulating DNA damage. Together, these consume NAD+ faster than the body can replenish it through salvage pathways. Human NAD+ levels drop ~50% between age 40โ60.
Two primary delivery routes exist: (1) intravenous injection at 250 mg โ 1 g over 2โ8 hours for maximum bioavailability, and (2) subcutaneous or intramuscular injection at 50โ100 mg per dose. Oral NAD+ itself has poor bioavailability โ oral precursors NMN and NR are studied at 250โ1000 mg/day.
